Some patients have difficulty maintaining their health once they leave the hospital. They may end up back in the hospital because of chronic illnesses or other conditions related to their previous stay. Hospital readmission has been a problem for some time across the nation, but the Patient Navigation program helps close gaps in care and eliminate barriers to care for Medicare patients who are most at risk for readmission.
Patient Navigators help patients take control of their own health by providing education and resources. They are involved in discharge planning, helping patients get the education they need, particularly on medications. The navigators stay in touch by phone, answering questions and making sure patients stay healthy.
Patient Navigators are part of Enspire Quality Partners’ overall system designed to improve quality, reduce costs and help you achieve your personal health goals.
